Connecting Communities and Healthcare: Making Social Prescribing Work for Everyone

The National Lottery Community Fund ( TNLCF) Learning Report on Social Prescribing

A new report from the Lottery on Social Prescribing has just been launched.

There is a local interest in the report as it mentions the Time to Shine Project (which is led by Leeds Older People’s Forum) and how their  experience is reflected in the sentiment of the report. In particular a brilliant case study from Age UK Leeds.

There is also  a case study on page 13 from the SWIFt service (formally known as Supporting Wellbeing) which gives a brilliant example of the ‘systems’ working together to create positive outcomes for the individual.
